Talak tiga means the husband has pronounced talaq three times, whether in one continuous marriage or across earlier reconciliations. After the third talaq, the divorce becomes final, and the couple cannot simply reconcile or remarry each other.

A common pattern: a husband pronounces talaq once, the couple reconciles, they argue again and he pronounces a second talaq, and they reconcile again. If he then pronounces a third talaq, that divorce is final with no ordinary path back.
